Acknowledgment

School officials, employees, and volunteers, are required to sign and acknowledge that the Administrative Regulation 1240 has been read and understood.

An example of an acknowledgment form follows:

I, _____________________________, acknowledge that I have received, read, and understood AR 1240, regarding confidentiality and prohibiting disclosure of confidential information. I further understand that willful disclosure of confidential information, as described in that AR 1240, may result in discipline up to and including termination. If I am a volunteer, I understood that such disclosure may result in prohibition against any further school-connected volunteer activities.
